The brief

A community trauma services team was losing 20 minutes per session to paper-based outcome forms — and losing data when forms didn't make it back to the office. The brief was direct: replace paper, don't replace people.

The approach

1. 1.Spent two days shadowing practitioners. Designed the capture system around the actual rhythm of a session, not the abstract idea of one.
2. 2.Built on REDCap with bespoke front-end. Five minute session note remained a five minute session note. No login friction. Offline-first.
3. 3.Quietly added structured outcome data without changing the practitioner experience. The data layer is invisible to them — the analytics layer is visible only to service leads.

The result

Outcome data completeness rose from below 50% to above 90% in the first quarter. Practitioner time spent on admin was effectively unchanged — exactly the point.
